Nerve-sparing surgery is a specialized surgical technique that attempts to preserve the neurovascular bundles near the tissues being removed during radical prostatectomy.1 This procedure specifically aims to protect the cavernous nerve sheaths that lie slightly underneath and to the sides of the prostate gland, which are responsible for erectile function.2 The neurovascular bundles contain nerves, arteries, veins, and lymphatic channels that control blood flow to the penis and initiate erections.3 During a nerve-sparing prostatectomy, surgeons meticulously separate the prostate from these delicate nerve structures, using precise dissection techniques to minimize nerve damage while still achieving complete tumor removal.4 This approach is particularly valuable in prostate cancer treatment, where preservation of sexual function is a significant concern for patients’ quality of life post-surgery.5
Clinical Context
Nerve-sparing surgery is primarily used in the context of radical prostatectomy for localized prostate cancer treatment.1 Patient selection for this technique is critical and depends on several factors including preoperative erectile function, age, tumor characteristics, and cancer stage.2 The procedure is most appropriate for patients with organ-confined disease (clinical stage T1-T2) where the cancer has not extended beyond the prostatic capsule.3
Preoperative assessment typically includes digital rectal examination, prostate-specific antigen (PSA) levels, biopsy results, and often multiparametric MRI to evaluate the risk of extracapsular extension.4 According to current European Urology Association guidelines, nerve-sparing surgery should be performed when there is a low risk of extracapsular extension, though careful selection may allow its use in some higher-risk cases.5
The surgical approach can be performed through open retropubic, laparoscopic, or robot-assisted techniques, with the latter offering enhanced visualization and precision.6 During the procedure, surgeons must carefully identify and preserve the neurovascular bundles that run posterolaterally to the prostate. The dissection can be intrafascial (remaining internal to the prostatic fascia) or interfascial (between the prostatic fascia and lateral pelvic fascia), with the former offering maximal nerve preservation but potentially higher risk of positive surgical margins.7
Postoperatively, patients may experience temporary erectile dysfunction as the nerves recover from surgical trauma, with function potentially improving over 6-36 months.8 Studies have shown that men who undergo nerve-sparing prostatectomy have significantly better chances of regaining erectile function compared to non-nerve-sparing approaches, with potency rates of ≥50% reported in carefully selected patients.9 Additionally, some evidence suggests improved urinary continence outcomes with nerve-sparing techniques.10
