Definition

Jelqing is a manual penis stretching exercise that involves repetitive pulling and massaging of the penis tissues in an attempt to increase penile length and girth.1 The technique purportedly works by creating “micro-tears” in penile tissues that supposedly expand during healing, similar to muscle growth from weight training.2 However, this theoretical mechanism lacks scientific validation, as penile tissues contain smooth muscle rather than skeletal muscle.3 Jelqing is typically performed on a semi-erect penis using lubricant and specific hand movements that apply pressure from the base to the glans.4 Despite its popularity in online forums and social media, jelqing has not been substantiated by rigorous clinical studies as an effective method for penile enlargement.5

Clinical Context

Jelqing is not a clinically recommended procedure for penile enhancement.1 The practice is primarily self-administered by individuals experiencing “small penis anxiety” or “short penis syndrome,” psychological conditions where men perceive their normally-sized penis as inadequate.2 Medical professionals generally advise against jelqing due to potential complications including penile tissue damage, bruising, pain, skin irritation, and in severe cases, the development of Peyronie’s disease (characterized by painful curved erections due to scar tissue formation).3

For patients presenting with concerns about penile size, clinicians should first provide education about normal anatomical variations, as studies indicate the average erect penis length is approximately 5 inches (13 cm), with 95% of men having flaccid penis lengths ranging from 6 to 12.5 cm.4 Psychological counseling is the recommended first-line approach for addressing body image concerns related to penile size.5 For cases of true micropenis (stretched length <7.5 cm), medical professionals may consider evidence-based interventions such as penile traction devices or, in rare cases, surgical options.4
