Endothelial dysfunction is a pathological condition characterized by impaired functioning of the endothelium, the thin layer of cells lining the interior surface of blood vessels and cardiac chambers.1 This dysfunction results in abnormal regulation of blood vessel tone, causing blood vessels to constrict or narrow instead of dilate.2 The condition is primarily caused by decreased nitric oxide (NO) bioavailability in blood vessel walls, leading to impaired vasodilation, increased vascular inflammation, and altered vascular permeability.3 In the context of erectile function, endothelial dysfunction affects the small resistance arteries of the penis and the penile corpus cavernosum, disrupting the normal vasodilatory response necessary for penile erection.4
Clinical Context
Endothelial dysfunction is clinically significant in various vascular conditions, particularly in the pathogenesis of erectile dysfunction.1 The condition represents an early marker for atherosclerosis and is strongly associated with cardiovascular diseases, diabetes mellitus, hypertension, hypercholesterolemia, and metabolic syndrome.2 In the context of erectile function, endothelial dysfunction impairs the nitric oxide (NO)/cyclic guanosine monophosphate (cGMP) pathway, which is essential for cavernosal smooth muscle relaxation and penile erection.3
Clinically, endothelial dysfunction manifests as impaired vasodilation in response to endothelium-dependent stimuli. In erectile dysfunction, this results in insufficient blood flow to the corpus cavernosum, preventing adequate tumescence.4 The condition is particularly prevalent in men with diabetes, hypertension, and cardiovascular disease, where systemic vascular impairment affects penile vasculature.5
Treatment approaches target the underlying causes of endothelial dysfunction and include lifestyle modifications (smoking cessation, weight management, physical activity), medications to improve endothelial function (statins, ACE inhibitors), and specific treatments for erectile dysfunction such as phosphodiesterase type 5 (PDE-5) inhibitors, which enhance the NO/cGMP pathway.6
