Definition

Transcorporeal Resection, along with Optical Corporotomy, are techniques proposed for visually guided, minimally invasive, and force-free excavation of fibrosed corpora cavernosa. This aims at facilitating easier penile prosthesis implantation with fewer complications and improved outcomes. These techniques are particularly relevant in cases where the corpora cavernosa are affected by extensive fibrosis, which can result from conditions such as neglected or irresolvable ischemic priapism, delayed re-implantation after an infected prosthesis removal, penile irradiation, or severe Peyronie’s disease. Traditional methods often involve sharp dissection and forceful dilation, leading to risks like urethral or tunica albuginea perforation and increased infection rates. The transcorporeal approach, as described by Shaeer et al. (2007)1, offers a safer alternative by allowing for direct visual monitoring during the excavation of fibrous tissue, thereby minimizing blind instrumentation and extensive manipulation.

Clinical Context

Transcorporeal excavation, particularly Shaeer’s punch technique, is primarily employed in the clinical management of severe penile fibrosis, often encountered in patients requiring penile prosthesis implantation. This fibrosis can stem from various conditions, including neglected ischemic priapism, complications following the removal of an infected penile prosthesis, penile irradiation, or extensive Peyronie’s disease. In these scenarios, the fibrosed corpora cavernosa present a significant challenge to successful prosthesis implantation due to the difficulty in creating a suitable tunnel for the device. Traditional methods, relying on blind dissection and forceful dilation, carry high risks of complications such as urethral perforation, tunica albuginea injury, and increased infection rates.

Shaeer’s technique offers a minimally invasive and visually guided approach to excavate Peyronie’s plaques and fibrotic tissue from within the corpora cavernosa. This method aims to reduce the complications associated with conventional techniques by allowing for precise, force-free tissue removal under direct visualization. Patient selection for this procedure typically includes individuals with severe corporal fibrosis who are candidates for penile prosthesis implantation. The expected outcomes include safer and easier implantation of the penile prosthesis, potentially leading to improved patient satisfaction and reduced post-operative complications compared to traditional methods. The technique’s efficacy in addressing challenging cases of penile fibrosis underscores its importance in modern urological surgery.2
