This is typically achieved through a minimally invasive office-based procedure involving puncture and drainage of the device’s fluid components.1 The primary purpose of deactivation is to avoid more complex surgical explantation procedures, especially in patients who are at high surgical risk due to multiple comorbidities or severe cognitive decline, or in cases where the patient or their caretaker desires removal of a normally functioning, well-positioned, and uninfected implant.1 The procedure aims to permanently disable the implant, preventing unintended inflation or discomfort, and is considered a safe and feasible alternative to surgical removal in specific patient populations.1

Penile prosthesis deactivation via puncture and drainage is clinically indicated for specific patient populations where surgical explantation of an inflatable penile prosthesis (IPP) is considered high risk or not desired. This procedure is typically considered for patients with multiple comorbidities that elevate their surgical risk profile.1 It is also an option for patients with severe cognitive decline, such as dementia, where the continued presence of a functional implant could lead to complications or distress, and for whom sexual activity is no longer a concern.1 In such cases, consent is often obtained from caretakers or legal guardians.1 Furthermore, deactivation may be performed for patients who develop an immutable aversion to their implant, even if it is functioning correctly and is not infected.1 The procedure is performed in an office setting and is considered when the implant is in a good position, cycles well, and there are no signs of infection.1 The expected outcome is the permanent non-functionality of the IPP, thereby preventing unintended erections or device-related issues without the need for more invasive surgery.1 Studies have shown this method to be safe and feasible, with no significant complications reported in early follow-up periods for appropriately selected patients.1
