Definition

A penile clamp is an external urinary incontinence management device designed to compress the penis and urethra to prevent involuntary urine leakage in men.¹ It functions by applying controlled pressure to the urethra, effectively occluding the urethral lumen and preventing urine flow until the device is released for voluntary urination.² The device typically consists of a hinged or flexible mechanism with soft padding that encircles the penile shaft, with various designs featuring different closure mechanisms and pressure distribution systems.³ Penile clamps are primarily used as a conservative management option for male stress urinary incontinence, particularly following prostate surgery or in cases where other interventions are contraindicated or have failed. These devices are designed to be removable, allowing for periodic urination and repositioning to maintain proper circulation and prevent tissue damage.

Clinical Context

Penile clamps are primarily indicated for the management of male stress urinary incontinence (SUI), which affects approximately 1-3% of the male population.¹ The most common clinical scenario for their use is in post-prostatectomy incontinence, where studies indicate that 60-82% of patients experience some degree of stress incontinence immediately following radical prostatectomy, with 5-10% experiencing persistent long-term incontinence.² Other surgical procedures associated with male SUI include transurethral resection of the prostate (TURP) and holmium laser prostate enucleation (HoLEP), though these procedures have lower rates of long-term incontinence (0.5% and 1.4-44%, respectively).²

Patient selection criteria for penile clamp use include:

  • Men with mild to severe stress urinary incontinence³
  • Patients with adequate manual dexterity to operate the device
  • Individuals with normal penile skin integrity (absence of severe penile skin disease)
  • Patients without artificial urethral sphincters
  • Individuals without psychosis or psychiatric symptoms that would impair proper device use

Penile clamps are typically recommended as a conservative management option before considering more invasive surgical interventions such as artificial urinary sphincter implantation or male sling procedures.¹ They are particularly valuable for temporary incontinence during the recovery period after prostate surgery or as a long-term solution for patients who are poor surgical candidates.

Clinical studies have demonstrated that penile clamps can significantly reduce the daily usage of urinary care pads. In one randomized prospective study, patients using the CLAMPMED® device showed a significant reduction in pad usage compared to non-clamp users (-0.83 ± 1.51 vs. -0.16 ± 0.69, P = 0.0071). However, the same study found that while the device reduced the amount of urinary incontinence, it did not significantly improve quality of life scores as measured by the King’s Health Questionnaire.

For optimal safety and efficacy, clinical guidelines recommend:

  • Using the clamp only during daytime hours
  • Taking 15-minute breaks every 2-3 hours
  • Repositioning the device regularly to prevent tissue damage³
  • Monitoring for signs of penile inflammation or circulatory compromise

While penile clamps have demonstrated effectiveness for stress incontinence, their utility is more limited for urge incontinence, as they do not eliminate the urge to urinate, though they may help patients reach the bathroom in time.²
