Phantom Technique (Infrapubic) Penile Implant
A well-established surgical approach for penile prosthesis placement, using a small incision in the pubic hairline to provide direct anatomical access for the surgeon.

What is the Phantom Technique?
The infrapubic (IP) approach is a standard, widely used surgical technique for implanting a penile prosthesis. The term "Phantom Technique" is a patient-friendly name for this same procedure. The surgeon makes a small incision just above the base of the penis, concealed within the pubic hairline, to place the components of the penile implant.
Also Known As
- Infrapubic (IP) Approach
- Suprapubic Approach
- "Phantom Technique" (Patient-friendly term)
Device Options
This approach is suitable for implanting a three-piece inflatable penile prosthesis (IPP), such as the Infla10® DIPP (Dynamic Inflatable Penile Prosthesis). Your surgeon will recommend the best device for your specific anatomy and clinical needs.
Is this for me?
According to the EAU Guidelines, a penile prosthesis may be considered for men with erectile dysfunction (ED) who have not had success with other medical therapies. Your urologist will discuss all suitable options with you so you can decide together.

Are not suitable candidates for or have not had success with other medical therapies
You've tried other ED treatments such as oral medications or injections but they haven't provided the results you need, or they're not appropriate for your situation.

Prefer a permanent, long-term solution for erectile dysfunction
You're looking for a permanent solution that provides reliable erections on demand and gives you control over when and how long you have an erection.

Have been thoroughly counseled on the risks, benefits, and realistic outcomes
You understand this is a surgical solution and want to explore all treatment options through shared decision-making with your healthcare provider.
How the Procedure Works
The infrapubic approach involves several carefully planned steps performed by your surgeon under anesthesia.
Anesthesia & Prep
You will receive general or spinal anesthesia. The surgical area is thoroughly cleaned to ensure sterility.
Small Hairline Incision
A short incision (typically 2-3 cm) is made just above the base of the penis, within the pubic hairline.
Corporal Access & Sizing
The surgeon accesses the erectile bodies (corpora cavernosa) and measures them to select the appropriate cylinder size.
Device Placement
The inflatable cylinders are placed inside the penis. The reservoir is placed in the space of Retzius, and the pump is placed in the scrotum.
Closure
The incision is closed with sutures, and a surgical dressing is applied. You will receive detailed post-operative instructions.
Surgical techniques vary. Your surgeon will tailor the procedure to your specific anatomy, medical history, and the chosen device.
What to expect
Surgery day
Usually short stay (per center protocol). Most procedures are performed as day surgery or with a short hospital stay.
Week 1-2
Light activities often resume in 1-2 weeks (as advised). Back to light activity, as guided by your medical team.
~6 weeks
Many people are cleared for sexual activity at about 6 weeks after their follow-up. Your surgeon decides.
Risks
As with any surgery, risks include infection, bleeding/pain and device issues; sometimes revision is needed. We work with your surgeon to minimize risks. Don't worry—the Rigicon Infla10® Pulse™ family is backed by a Product Replacement Lifetime Warranty for your peace of mind.*
*Recovery is individual. Always follow your surgeon's advice.
Phantom Technique (Infrapubic) vs. Penoscrotal: A Comparison
Both the infrapubic and penoscrotal approaches are standard, effective techniques for penile implant surgery. A 2020 systematic review cited in the EAU Guidelines found no significant difference in satisfaction or complication rates between the two. The best approach depends on the surgeon's training and experience, as well as the patient's anatomy and surgical history.
Feature | Phantom Technique (Infrapubic) | Penoscrotal Approach |
---|---|---|
Incision Location | Above the penis in the pubic hairline | On the scrotum |
Surgical Access | Direct access to corporal bodies and reservoir space | Direct access to pump placement area |
Incision Concealment | Hidden within pubic hair | Located on scrotal skin |
Surgeon Experience | Widely used technique by experienced urologists | Also widely used with excellent outcomes |
Safety Profile | Excellent safety when performed by experienced surgeon | Comparable safety profile |
Satisfaction Rates | High patient satisfaction rates (92-100%) | Similar high satisfaction rates |
Infection Risk | No significant difference compared to penoscrotal | No significant difference compared to infrapubic |
Best for | Patients preferring pubic hairline incision concealment | Depends on surgeon expertise and patient anatomy |
Key Takeaway: According to major urological guidelines, neither approach is definitively superior. Both have comparable safety profiles and high satisfaction rates when performed by an experienced surgeon.
Frequently Asked Questions
Is the "Phantom Technique" a new or different surgery?
No. "Phantom Technique" is a patient-friendly term for the standard infrapubic approach, a technique that has been used by urologists for many years.
Is the infrapubic incision truly invisible?
The incision is placed within the pubic hair, which typically conceals it well. However, scar visibility depends on individual healing, hair patterns, and skin tone. Your surgeon will discuss realistic expectations.
Which approach is safer or better?
According to major urological guidelines, neither approach is definitively superior. Both have comparable safety profiles and high satisfaction rates when performed by an experienced surgeon. There is no strong evidence that one approach has a lower infection risk than the other.
How long do penile implants last?
Modern inflatable penile prostheses are durable. A large meta-analysis reported implant survival rates of approximately 87% at 5 years and 77% at 10 years. Revision surgery may be needed eventually.
Will an implant affect sensation, orgasm, or ejaculation?
A penile implant restores the mechanical function of an erection. It does not typically interfere with skin sensation, the ability to achieve orgasm, or ejaculation, assuming these were normal before surgery.
