I remember it vividly\u2014twenty years ago<\/strong>, you came to the\u00a0University of Rome<\/strong>\u00a0to perform\u00a0three penile implant surgeries<\/strong>. I was very young at the time, and watching you work had a profound impact on me.<\/p>\n

That was the moment I knew\u00a0this would be my path<\/strong>.<\/p>\n

For our audience, could you explain\u00a0what a penile implant is<\/strong>\u00a0and how the field has evolved over time?<\/p>\n

Prof. Steven K. Wilson:<\/strong><\/h3>\n

That\u2019s a great question, and this is the\u00a0perfect time<\/strong>\u00a0to discuss it because this year marks the\u00a050th anniversary<\/strong>\u00a0of the\u00a0three-piece inflatable penile prosthesis<\/strong>.<\/p>\n

Fifty years! Professor Wilson tells us that\u00a0penile implants have now been on the market for half a century<\/strong>.<\/p>\n

Yes. The\u00a0first penile prosthesis<\/strong>\u00a0was developed in\u00a01973<\/strong>\u00a0by a\u00a0urology professor at Baylor University in Houston, Texas<\/strong>.<\/p>\n

The first-generation implant was\u00a0made entirely of silicone<\/strong>, but it had\u00a0many flaws<\/strong>. It required a\u00a0large abdominal incision<\/strong>\u00a0because the\u00a0connecting tubes<\/strong>\u00a0had to be inserted through the\u00a0inguinal canals<\/strong>.<\/p>\n

The procedure was\u00a0complex<\/strong>, and the device\u00a0frequently broke<\/strong>.<\/p>\n

When exactly was this?<\/p>\n

Prof. Steven K. Wilson:<\/strong><\/h3>\n

1973<\/strong>. At that time, I had just finished my residency and was looking for a\u00a0specialty to focus on<\/strong>.<\/p>\n

I attended the\u00a0first training course<\/strong>\u00a0on penile implants, and in\u00a01974<\/strong>, I performed my\u00a0first implant surgery<\/strong>.<\/p>\n

But back then, the device was\u00a0far from perfect<\/strong>. It wasn\u2019t truly effective until\u00a0the mid-1980s<\/strong>, when\u00a0significant mechanical improvements<\/strong>\u00a0made it\u00a0reliable<\/strong>.<\/p>\n

2. The Advancements in the 1980s and 1990s<\/strong><\/h2>\n

Prof. Steven K. Wilson:<\/strong><\/h3>\n

By the\u00a0mid-1980s<\/strong>, we saw a major\u00a0breakthrough<\/strong>. The\u00a0mechanical reliability<\/strong>\u00a0of penile implants\u00a0improved significantly<\/strong>, making them a\u00a0viable long-term solution<\/strong>\u00a0for patients.<\/p>\n

At that time,\u00a0two major companies<\/strong>\u00a0were leading the field:<\/p>\n

  • AMS (American Medical Systems)<\/strong>\u00a0\u2013 The original manufacturer of penile prostheses, founded in\u00a01973<\/strong>.<\/li>\n
  • Mentor<\/strong>\u00a0\u2013 A competitor that entered the market in\u00a01983<\/strong>, bringing further innovation.<\/li>\n<\/ul>\n

    This competition led to\u00a0continuous improvements<\/strong>\u00a0in the durability, design, and effectiveness of penile implants.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    Then, for nearly\u00a030 years<\/strong>, there were\u00a0no new players<\/strong>\u00a0in the market\u2014until\u00a02019<\/strong>, when\u00a0Rigicon<\/strong>\u00a0introduced their own prosthesis.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    3. Surgical Techniques: Penoscrotal vs. Infrapubic Approach (Phantom Technique)es<\/strong><\/h2>\n

    Now, let\u2019s talk about\u00a0surgical techniques<\/strong>. There\u2019s the\u00a0penoscrotal approach<\/strong>\u00a0and the\u00a0infrapubic approach<\/strong>.<\/p>\n

    Which one do you prefer?<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    In the\u00a0early days<\/strong>, everyone used the\u00a0infrapubic approach<\/strong>, because it was the\u00a0original technique<\/strong>\u00a0developed by\u00a0Dr. Scott<\/strong>.<\/p>\n

    That was the\u00a0standard method<\/strong>\u00a0until\u00a01981<\/strong>.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    Dr. Scott was performing a\u00a0penile implant surgery<\/strong>\u00a0on a patient who had undergone\u00a0five previous abdominal surgeries<\/strong>. During the procedure, he encountered a\u00a0serious complication<\/strong>.<\/p>\n

    This forced him to think of a\u00a0new way to insert the implant<\/strong>, leading to the invention of the\u00a0penoscrotal approach<\/strong>.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    After that, the\u00a0penoscrotal approach<\/strong>\u00a0became the preferred method because it was\u00a0easier for the surgeon<\/strong>. However, it was\u00a0not necessarily better for the patient<\/strong>.<\/p>\n

    Dr. Gabriele Antonini:<\/strong><\/h3>\n

    Professor Wilson explains that\u00a0early penile implants required large abdominal incisions<\/strong>.<\/p>\n

    Because this was\u00a0too invasive<\/strong>, surgeons switched to a\u00a0penoscrotal approach<\/strong>, which was\u00a0easier for doctors<\/strong>\u00a0but\u00a0more painful for patients<\/strong>.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    Then, in the early\u00a02000s<\/strong>,\u00a0Dr. Paul Perito<\/strong>\u00a0revolutionized the field by\u00a0modifying the infrapubic approach<\/strong>\u00a0to be\u00a0minimally invasive<\/strong>.<\/p>\n

    At that time, about\u00a085% of surgeons<\/strong>\u00a0used the\u00a0penoscrotal approach<\/strong>. But today, the numbers have shifted to\u00a065% penoscrotal and 35% infrapubic<\/strong>.<\/p>\n

    Prof. Steven K. Wilson:<\/strong><\/h3>\n

    However,\u00a0learning this technique is difficult<\/strong>. It\u2019s not something you can just\u00a0improvise<\/strong>\u2014it requires\u00a0proper training and expertise<\/strong>.<\/p>\n
